Waiting Well Connector

 

 

Hours:            24 hrs per week

 

Salary:           £ 13,418 per annum 

 

Fixed term contract length: 24 Months

 

 

The Waiting Well Connector will provide nonclinical support to people awaiting orthopaedic surgery helping them to arrive for surgery and any follow up appointments in the best possible physical and mental health.

Research shows that people who keep mentally or physically active feel happier and live healthier. Patients taking advantage of this service can be helped to maintain and improve overall wellbeing and have a faster recovery post-surgery.

The Waiting Well service works alongside medical professionals and support agencies across Great Yarmouth and Waveney accepting referrals for patients identified as needing additional support. Support could include advice and advocacy on welfare related matters, connecting them to community-based networks of support and/or activities and encouraging people to remain safely active and take steps to live healthy fulfilled lives.

Key Responsibilities

1.   To compliment medical intervention, using people’s existing skills and assets to expand upon and support new networks to increase personal resilience.

 

2.   Provide first step information, advice and guidance, and support residents to take up activities within the community.

 

3.   To access identified support services through supportive, personalised introductions.

 

4.   Provide vital links to voluntary and community base activity and capacity building groups.

 

5.   Ensure comprehensive links with wider support infrastructure across the locality.

 

6.   To have up to date local knowledge of other services being provided within the locality to ensure the varying needs of individuals  are met.

 

 7.   Actively participate and contribute in local inter agency support settings including

 

8.   Collect and produce evidence of support given to individuals for monthly line manager meetings.

 

9.   To always practice in an anti-discriminatory and professional way, always considering the safeguarding and dignity of vulnerable individuals.

 

10. To establish working relationships with individuals based on trust and mutual respect.

 

11. To manage and maintain caseloads to meet targets as agreed with managing organisation and as required by funder and early help and localities leader group.

 

12. To maintain effective recording, monitoring and evaluation systems of individual files ensuring confidentiality is maintained in accordance GDPR requirements.

 

13. To be available to work evenings and weekends as and when required.
