Job Title: Multi-Disciplinary Support Worker
Reporting to: CEO, DIAL Great Yarmouth
Main Relationships: DIAL Great Yarmouth
Hours of Work: 21 hrs per week
Salary: £11629.80 pro rata (FTE £20500), per annum. Fixed Term Contract for 12 months
Key Purpose:
The Multi-Disciplinary Support Worker will support new and existing DIAL clients, as part of DIAL’s multi-disciplinary team. This includes providing advice and support to beneficiaries, diagnosing issues and connecting people into appropriate specialist internal and external support networks.
The post holder will connect with communities in face to face settings at DIAL’s Kingside base whilst also working alongside our partner organisations in various community settings to maximise the reach and impact of DIAL services.
Key Responsibilities:
· To work closely with the DIAL specialist services to provide advice and guidance to the most vulnerable residents within Great Yarmouth.
· To support client referrals to other local providers as and when their needs are identified.
· To work with other organisations to increase and raise awareness of services that are provided within Great Yarmouth, including organisations from the public, private and voluntary sectors, including services provided by communities themselves.
· To form working relationships with communities by undertaking outreach at a range of multi-agency hubs, including neighbourhood-led and early help hubs.
· To provide specialist advice and support on evenings and weekends as required, in accordance with the identified needs of clients
· To provide general advice and guidance on all housing matters.
· To carry out agreed assessments and reviews using an agreed monitoring tool as required by the project, maintaining an up- to-date data management system.
· To network and liaise with other statutory and voluntary organisations, actively participating in front line practitioner meetings, training and programme development workshops.
General:
· To adhere to the policies and procedures of DIAL Great Yarmouth.

· To attend training events for both the project and DIAL as and when required.
